You may register with us by completing the online Patient Registration Form. Please make sure to click on the "email button" at the bottom after completing the form. The form will be emailed to our address at dc.nova.smiles@gmail.com. When we see you for your appointment, we will have the forms ready for your signature. If you would like to download the forms to complete them by hand, click on the "download" button.

Our new patient form and email links are offered as conveniences for our website visitors. We recommend that you fill out the form, print it, and bring it to your appointment. Patients who chose to email us in any capacity and through the use of any means available on this website assume all risk associated with sending any and all materials over the Internet. Financial information

Schedule your initial consultation by calling DC Smiles in Washington, DC or Alexandria, VA. To assist you in achieving a healthy, beautiful smile, whether through restorative or cosmetic dentistry, we offer third-party financing through CareCredit, Springstone and MedChoice Financial. Apply for financing online or call them to apply by phone. The payment plans offered by these companies allow you to start treatment immediately and pay over time with low monthly payments. Choose from no- and low-interest payment plans.

Payment is due at the time of service. We accept cash, personal checks, VISA, MasterCard, American Express, and Discover.

DC Smiles does not participate in HMO or PPO insurance plans. We find that insurance companies pay for average dentistry and lab work. Our practice provides services and materials of a higher quality than average. In our dedication to excellent customer service, we will file your insurance claims and collect for the services at the time services are performed. We are committed to assisting you in maximizing your insurance benefits to the fullest extent.
